Renal Nutrition Therapy: Beyond the Basics

Episode Description:

Traditionally, dietary interventions for CKD have emphasized limiting protein, phosphorus, potassium, and sodium intake to slow the progression of renal failure and reduce cardiovascular risks. However, emerging research highlights the importance of a personalized approach that considers individual dietary responses, disease stage, and comorbidities such as diabetes and hypertension.

Low protein diets (LPDs) are standard, but the degree of restriction may vary based on patient tolerance, nutritional status, and CKD stage. Very low protein diets (VLPDs) with keto acid supplements can benefit patients in advanced stages of CKD.

Phosphorus control is critical due to its impact on bone and cardiovascular health. Modern approaches differentiate between organic phosphorus (found in natural foods) and inorganic phosphorus (found in processed foods), with the latter being more readily absorbed and contributing to hyperphosphatemia.

Potassium management is complex, requiring tailored advice based on serum potassium levels, CKD therapy type, and medications. Both hyperkalemia and hypokalemia are risks in CKD.

Micronutrient management, including antioxidant supplementation and vitamin D correction, plays a significant role in reducing oxidative stress, inflammation, and cardiovascular risks.

Dietary patterns, such as the Mediterranean diet, which is rich in fruits, vegetables, whole grains, and fish, have shown benefits in reducing inflammation and improving kidney health. These diets emphasize natural foods over processed ones, promoting better overall health outcomes.
